Asking a provider to whitelist you? How?

I saw a provider that has an account on TER.  Do I just message her on TER and ask her to whitelist me?

...or is there a some type of whitelist request button?

I almost reported this post as a duplicate and to have it removed.

You could have asked her before you ended your date.  Yes, you can message her and ask her, but you will have to make sure she knows who you are.  So tell her when you visited her, your handle if she knows it.

No, there is no button, there is a simple procedure which is explained in the self help center.

are not always worth the paper they are printed on. For instance, a whitelist from a back page provider with zero, minimal, or mediocre reviews will not help you very much.

I have only White listed 4 people... But the oldest is from 2011, another feb 2012, and the last two within 2 weeks...  

How long should I keep the White list active?  ALWAYS? or?

All the ladies I've been seeing have TER profiles/reviews but I've never asked them about Whitelisting.
They have afterwards recommended me on Date-Check so I have them as references which would help if I see a non-TER lady. Even if I see a TER lady I don't see the need for a whitelist now since I already have some recent reviews and current references if needed.
