As to body types, you hit the proverbial nail on the [big?] head: "most providers cease to be surprised by the variation in ages, body types, ethnicities, etc. of the men who come to see them."
As to the YMMV issue - I think you are describing what we in psychodynamic therapy would call "the intersubjective space." This means that there is never just ONE person involved in the encounter - it's two people, and they surely are making different chemistry for every unique coupling out there. And, seeing that providers are women first and providers second - well, that simply means that body odor, body type, individual preferences MAY influence their behavior toward the hobbyist. And of course, the hobbyist then reacts to the provider's reaction...and bingo, we either have Mozart or Metallica, depending on what flava the encounter takes on.
On the other hand, as you so succinctly point out: "providers either develop a certain level of clinical detachment or they come to understand the human body so well that they are rid of inhibitions and preconceptions." Bingo! Yes on both counts. This means that providers are professionals, and they share this with anyone whose job it is to deal with human bodies - physicians, nurses, chiropractors, morticians, etc.
In the final analysis, however, I would say that the YMMV may rest more on how the man treats the lady, and her subsequent reactions in the bedroom. Then again, there is the whole topic of hobbyists having unrealistic expectations; with such factors in play, the lady's feelings and behavior may not even enter the equation.

I have to say, as a provider, I was a nurse before starting massage, so nothing surprises me anymore. I can enjoy myself with all body types. On the other hand, as a provider, I have had my first person come to my incall and then say I just wasn't his type and I told him that was ok and he left. It goes both ways. I don't think everyone is used to the human body so much. It depends on who you are seeing. If it bothers you or you are worried it will bother them, just ask. There is no shame in it.

Never thought of the mortician commonality you did. Wonder if dealing with strange men's bodies feels that way, sometimes, to providers.
Both professions have to work with big stiffs.
but many providers have pointed out to me that men and women are different. Beautiful women seem to be able to deal with --indeed, fall in love with--men that don't hold a candle to them in the looks department. One ATF--a beautiful woman with a killer body--told me about a married boyfriend she had who was short, fat and bald, whom she would have married had he left his wife. Personally, I am old, fat and not very handsome, but I have close personal relationships with beautiful, shapely and top-notch providers. Go figure.
