No, a photograph is not required on the provider's web site.

According to the ad posting guidelines (link below), only two things are required to be on a provider's web site if she wishes to link to it in her TER ad posts:  1) contact information, and 2) the TER Site Reviews logo (with working link) on the first or second page of the web site.

Photo images are not stored on TER servers, so you cannot upload photographs.  But you may link to a photograph stored elsewhere; simply place the link in the 'picture URL' box of the post message screen.  Here is a link on how to capture photo URLs:
