1)  There are large brothels (many girls to choose from) and there are rural brothels (not so many girls to choose from).  Pick your poison.  Each house seems to have it's own vibe.  Experiencing a brothel line-up is for sure something to enjoy.  But you can also mingle in the bar, meeting multiple providers before choosing to take a tour (slang for let's go talk money).  There may be 2 entrances to a brothel...the door to the bar, or the ring the bell for a line-up door. With most brothels the business plan is that the provider splits the fee with the house, so it could very well be more expensive than an escort.  Fees will not be discussed period until you are in her room negotiating.  Some brothels have a minimum, and will not allow a gal to book a party for less.  
2)  All services in a brothel are supposed to be covered.  That's the law.  There are no cameras watching what goes on BCD, so maybe YMMV.  
3)  Yes.  But expect a service fee to be attached in most locations.

I went to a brothel outside of Vegas a few years ago. All in all it was a fun experience, but once was probably enough. Did it more as a bucket list thing.  

I called Sherri’s Ranch which is about 60-70 miles outside of Vegas. They have drivers that will come and pick you up at your Casino and drive you to and from. But you’re supposed to tip them about $60-$80, it’s at least an hour ride each way. The drivers aren’t really supposed to give you any information about the girls, but mine was nice enough to give me a few tips.

I'm late to respond to the OP's questions about the brothels, but just to add some intel for others who might be interested in the NV brothels I'll add to what has already been mentioned.  I was a long time patron of the many of the NV brothels until the pandemic hit.  After the brothels reopened, rates have gone up everywhere which isn't surprising.   From my reliable sources, you'll be paying low-mid 4-figures for an hour of basic services at the larger brothels like Sheri's Ranch and Chicken Ranch.  The Chicken Ranch use to be more affordable than Sheri's, but it's not anymore.  Those are the only 2 brothels near Vegas.

Up north near the Carson City/Reno area, the Bunny Ranch is the highest priced of all the NV brothels with the Sagebrush not far behind.  The same with the Mustang Ranch.  Those are the only 3 brothels currently open in northern NV.  

The rural brothels are lower priced, but since they are located some distances from a major city, many don't want to drive that far unless they happen to be passing through those areas.  For example, Elko has 4 small brothels in town, but it's a 4 hr drive from Reno and even further if you're coming from Vegas.

Anyways if you act like a gentleman and are good at negotiating, you might get lucky, but you'll probably end up getting priced walked for anything under 1K at the larger brothels for basic activities.  The way the brothels have changed over the years with inflated pricing, it's probably best for those who have it on their bucket list or for someone who really needs to play legally, otherwise it's the worse value when it comes to P4P.  Personally, I prefer to see Indies whenever I visit Vegas now and I only visits some of the rural brothels if I happen to be passing through those areas for other reasons.

Having said that, there's nothing wrong with going to a legal brothel just to have the experience of going to one, which is exactly the reason why my friends and I went all those moons ago.

I would say keep your expectations in check, especially if you've been to similar establishments in Europe or Asia, and set a ceiling of what you're willing to spend and stick to it, as there aren't any set-in-stone menu prices and you will be high-balled with the expectation that you will negotiate downwards like you were souvenir shopping in Tijuana.

With that in mind, you should be in for a memorable time at least lol

In Vegas the Chicken Ranch and Sheri's are right next door to each other.   Sheri's has more women working, and typically hotter women, and higher prices. Last time I went the lineup had over 20 women during the afternoon.  Chicken Ranch had about 12.  At one time there was two others Love Ranch South and Alien Cathouse.  LR South is closed, not sure about Alien Cathouse.    If Alien Cathouse is still open you might want to try there too.
